Preferably based in New York City or Long Island.MAIN FUNCTION:

To provide assessment for those seeking help for Substance Use Disorder. Obtaining necessary information to determine patients needs, level of care and assist in accessing treatment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provider Relations liaison to Seafield Inpatient and Outpatient Programs and affiliates.

  • Provide clinical assessment for potential referrals.

  • Coordinate with Intake, transportation, patients and referral sources.

  • Participate in marketing meetings.

  • Participate in weekly Call-in, submit monthly Expense reports.

  • Advocate for the well-being of our patients and their families.

  • Perform CD intake screenings including gathering demographics and insurance information for input in AVATAR.

  • Provide thorough assessments to determine the appropriate admission level of care. Input all collected information into AVATAR. Coordinate with supervisors, family, treatment, and referrals sources to provide the most appropriate level of care.

  • Complete all required documentation for assessments and provide direct referrals to other providers for patients not entering the Seafield system.

  • Complete weekly reports, document screenings and assessments conducted, as well as outcomes.

  • Attend all department meetings and when requested, assist with outreach efforts to establish and maintain relationships with referral sources.

  • Available for: in person, zoom, and telephonic assessments that may include travel to various work sites as deemed appropriate and necessary.

  • Represent the agency in appropriate community education and outreach activities.

  • Perform other tasks and responsibilities as needed by the supervisor and Director of Department.

  • Collaborate with other professionals involved with patients, when appropriate.

  • Work with families and referral sources to meet the needs of each in the assessment process.

EDUCATION AND QUALIFICATIONS:

CASAC or CASAC Eligible, or B/A-B/S with working knowledge of Substance Use Disorders. Working knowledge of Electronic Medical Records and Managed Care.

  • While performing this job, the employee is required to talk and hear. Specific vision abilities required by this position include close vision, distance vision and the ability to adjust focus.

  • This is a largely sedentary role and involves sitting most of the time. Walking and standing are required occasionally.

  • The employee must have the ability to use a computer keyboard, mouse, calculator, copier, fax machine, scanner and telephone.
