Montefiore Medical Center ASSISTANT DIRECTOR NETWORK CARE MGMT in Bronx, New York

The Assistant Director (AD) of Care Management leads a team of case managers coordinating discharge planning. The AD facilitates and ensures effective throughput, identifies barriers to discharge and reports variances and complex issues to senior leadership. The AD ensures that pertinent policies and procedures are followed with respect to all legal, regulatory, and accreditation standards.
The ideal candidate will have experience leading people, utilizing effective communication and active problem solving..
The Assistant Director of Care Management will report to the AVP of Operations.
The candidate for this position will:
Manage multiple projects in a fast paced environment
Work collaboratively with stakeholders across organizational units
Understand and use metrics to guide change
Conduct productive, orderly meetings and work sessions
Demonstrate the ability to work in matrix environment while aligning with organizational strategy
Foster and maintain strong relationships with leadership throughout Wakefield and MMC
Promote teamwork and a positive work environment
Education and Experience:
Bachelor's Degree required
Master's degree preferred
Current NYS Licensure as a Registered Nurse
At least five years of Case Management experience required
PRI certification required
CCM/ACM certification preferred
Knowledge required in EPIC, Kronos ,Allscripts, Microsoft Office, Clinical Guidelines (ie: Interqual, MCG)
