Description
  1. Maintain tracking of initial and annual mental health screenings and meet bi-monthly with medical secretary and mental health screeners to review process and workflow. Coordinate scheduling of mental health screenings with medical secretaries in both regions.
  2. Receive all mental health service referrals for both regions and distribute to appropriate “Team Supervisors/Clinical Coordinators” after referral forms and consents are received from case planning.
  3. Follow process of psychiatry referrals from receipt of referral form and consent to confirmation that evaluation occurred. Liaison with both regional psychiatrists and medical secretaries to facilitate the process.
  4. In collaboration with case planning staff, coordinate the Special/exceptional rate evaluation workflow including review of referral for such an evaluation and assignment to appropriate evaluator.
  5. Carry small caseload of Nurturing Parenting-Therapeutic Visitation cases and attend group clinical/model supervision.
  6. Carry small caseload of Nurturing Parenting-Therapeutic Visitation and co-lead “It’s All About Being a Teen” group.
  7. Attend staff, disposition and treatment planning meetings as needed.
  8. Complete, on a timely basis, all mental health treatment reports and progress notes.
  9. Perform initial mental health screening assessments for new admissions to foster care homes in the Bronx/North Manhattan region. Approximately 30% of the screenings are conducted via Telemental health.
  10. Collaborates with the medical staff on scheduling the mental health screenings at the same time as that of the initial medical appointment and conducts 70% of the onsite screenings at the medical clinic.
  11. Attend staff, disposition and treatment planning meetings as needed.
Qualifications
Education

Required

Master's Level Degree or better in Psychology or related field.

Master's Level Degree or better in Social Work or related field.

Master's Level Degree or better in Counseling or related field.
