We are currently seeking dynamic Senior Care Managers to join our Care Management Team in the Bronx.
In this role you will:
Provide intensive care management services to our most vulnerable clients which, are high risk and Health Home + (HH+) level. The Senior Care Manager will provide linkages to other providers, and ensures all providers are active participants in the members care planning. Additionally, the Senior Care Manager will ensure comprehensive and appropriate care needs are met to stabilize members and promotes access to health and wellness while reducing healthcare costs.
What qualifications do you need:
Bachelor's Degree in Human Services or related field such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing; with two years of experience; OR
Bachelor´s level education or higher in any field with three years of experience OR two years of experience as a Health Homes CM/PCN; OR
Master’s degree in one of the qualifying fields and two years of experience; OR Master’s degree in a related field may substitute for up to one year of experience.
A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with two years case management experience.
Related experience consists of providing direct services to persons with serious mental illness, or developmental disabilities, or alcohol and/or substance use disorders or linking persons who have serious mental illness (SMI) or developmental disabilities or alcohol and/or substance use to a broad range of services essential to successfully living in a community setting.
Bilingual-Spanish, strongly preferred.
EHR environment and proficiency in Microsoft Office Suite.
Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
Team player and able to work independently.
Willing to travel to members’ homes and within the community.
