The Brookdale Hospital & Medical Center CLINICAL COORDINATOR, QUALITY ASSURANCE in Brooklyn, New York
Manages and coordinates organization-wide efforts to ensure that improvement (PI) and quality improvement (QI) programs are developed and managed using a data-driven focus that sets priorities for improvements aligned to ongoing strategic imperatives. Assures that the area/departments assigned are focused and aligned on improving operational improvement and quality as well as overall program efficiency and effectiveness.
Provide leadership and coordination for improving the organization’s core measures and key performance indicators and evaluating the impact that systems improvements have on quality of care and publicly reported data; and researches and develops PI and QI training programs that focus on enabling the hospital community to achieve improvements with priority health focus areas.
Manages and ensures implementation and effectiveness of performance improvement systems.
Facilitates the implementation of performance improvement activities that leads to a positive and measurable impact on process on outcome measures.
Researches, designs and conducts appropriate organization-wide performance and quality training.
Assists departments to evaluate and improve effectiveness of their practices and resource use.
Establishes a continuous performance and quality improvement effort and monitoring and reporting system for assigned departments.
Performs ORYX/CMS data abstraction in the specified timeframes.
Conducts chart review for actual/potential quality of care issues.
Provide regular feedback of the status of performance and quality improvement efforts and impacts.
Coordinates with assigned project managers on identified projects or initiatives.
Research and recommend implementation of "evidence-based" performance measures
Centralizes and tracks/trends the hospital mortality review process.
Assists in the maintenance of physician quality management files.
Increases program interactions to address knowledge deficit on quality, regulatory, and performance improvement requirements.
Assists the Director with other activities as needed.
Other duties and projects as assigned.
Performs other duties as required.
Education:
- Bachelor’s Degree (BA/BS) with a preference for a graduate degree in a major healthcare-related field.
Experience:
Minimum three (3) to five (5) years experience in healthcare.
Experience in Performance Improvement and Quality Leadership desired.
Knowledge and Skills:
Knowledge of performance improvement methodologies, core measures abstraction, safety and reliability science.
Skilled in communicating effectively, facilitating group processes and training staff; preparing data analysis.
Ability to establish and maintain effective and productive working relationships with all employees, public and private organizations, and regulatory agencies.
Proficiency with Office PC's, Desktops, and Laptop software in addition to familiarity with Microsoft Office softwares to include Word, Excel, Outlook, etc.
Excellent ability to communicate effectively in English, both verbally and in writing.
Familiar with the operations of Hospital departments.
Licenses, Certifications:
- Current NY State RN License preferred.
Physical Requirements:
Position may require prolonged periods of standing, walking, talking, sitting, hearing, and bending throughout the work day.
May include some repetitive motions.
