Position Summary

TheTraining and Education Supervisorprovides clinical and administrative supervision for a team of Social Work Interns. TheTraining and Education Supervisorworks closely with their team to ensure consistent provision of high quality outreach, assessment, intervention, referral, linkage, monitoring and service planning for all persons served.Training and Education Supervisor tracks programmatic data reports to ensure success service delivery and builds relationships with Health Home network medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for all clients.

TheTraining and Education Supervisormaintains and cultivates existing relationships with area social work programs and Field Placement offices, and works with those programs to recruit and on-board appropriate Interns. Training and Education Supervisorprovides task and clinical supervision to the group of Interns, and completes evaluations as prescribed by the identified universities.

TheTraining and Education Supervisorworks as a vital part of the Training and Education Team within the Health Homes Care Coordination Department. They help to create, and modify training materials, and regularly facilitates teaching and training sessions for groups throughout the HHCC department. TheTraining and Education Supervisoruses their clinical background, and technical skill, to assist in the clinical and skill development of the larger team.


Essential Functions

  • Acts as a Training & Education resource for the HHCC Team. Participates in the creation of trainings, the modification of trainings, and acts as an educator and trainor during individual presentations. Provides ongoing assessment and evaluation of trainings and training methods to his or her direct supervisor, and works with the larger team to continue to enhance clinical and practical skill development.

  • Ensures clients are provided effective care coordination services according to program requirements by mentoring and monitoring the Intern cohort, and intervening when necessary. Monitoring will be achieved by utilizing management tools, databases and multiple Electronic Health Records systems. Develops and implements action plans to improve effectiveness and efficiency of Interns, while ensuring a safe learning experience for each. Meets with clients and conducts site visits as needed.

  • Provides clinical and administrative supervision to a team of Social Work Interns. Provides program orientation and training regarding department procedures and operations. Monitors Intern performance, leads individual and team clinical supervision meetings, assists Interns in problem solving to achieve client and program goals. Composes and delivers evaluations to appropriate universities.

  • Creates policies, procedures and workflows as needed, in conjunction with departmental management staff. Works to explain and enforce department policy surrounding intervention, practice and billing systems on his or her team.

  • Cultivates relationships with area Social Work programs. Ensures consistent and reliable communication. Screens and selects appropriate Interns for each term. Provides appropriate and helpful feedback consistently to both the individual Student Intern, and the corresponding University Program.

  • Implements structured quality improvement (QI) activities. Reviews client records, including databases and Electronic Health Records to evaluate effectiveness of services. Contacts and meets with clients to follow-up on Interns' service delivery. Documents QI activities. Provides feedback and training to Interns based on QI outcomes.

  • Provides structured crisis intervention services as necessary to address client needs and resolve problems to promote client wellness. Interfaces with lead Health Homes, emergency services, Call Centers and other providers as needed.

  • Performs other duties as assigned


  • Master's Degree required

  • Strong understanding of clinical supervision, and of client population. Excellent computer skills and attention to detail

  • 3 years of Masters Level SW practice required.

  • This position requires a minimum of 1 year supervisory experience.

  • Bilingual Spanish speaking candidates strongly encouraged to apply.

  • Understanding of NYC Resources a plus.

  • LMSW or LSCW required

  • SIFI Eligible required
