Long Island Select Healthcare, Inc. Manager of Care Coordination Services - RN in Central Islip, New York


Long Island Select Healthcare Inc. (LISH) is a comprehensive network of community health centers under a Federally Qualified Health Center (FQHC) model. We provide Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. LISH is currently seeking a Manager of Care Coordination Services - RN to directly impact patient care as a front line leader. You will be responsible for the leadership, development and oversight of Care Coordination and Virtual Care programs across locations. Experience in Outpatient Care Coordination required. Applicant must be able to work regular M-F full-time hours. This is an on-site position

General Duties Include

  • Provide supervision and mentorship to Chronic Care Management (CCM) team. Oversee, monitor, and develop the performance needs of assigned personnel.

  • Develop a positive culture to promote continuous growth and staff development

  • Provides clinical and community resources for CCM team to ensure high quality, coordinated services are delivered appropriately

  • Serves as a resource to staff on all training and continuing education and client wellness activities and education and develops and coordinates programming in response to the needs of management; staff; clients; and of specific sites.

  • Development and implementation of care coordination model in accordance with FQHC and PCMH standards, values and models

  • Serve as the local content expert on the care coordination program, including resources and supports available to patients

  • Review monthly data reports; identify strengths and areas of opportunity; develop and implement strategies to address identified opportunities

  • Participates in Organization-Wide quality improvement program. Develops, manages and ensures the implementation of and adherence to departmental policies, procedures and quality activities.

  • Presents and reviews quarterly program reports and quality data to Organization leadership as needed.

  • Implements and evaluates compliance to NYS DOH regulations. Participates in preparation for accreditation and program certification surveys. Involves staff at all levels in survey compliance, particularly as it relates to CCM and Virtual Care Programs.

  • Participates in the recruitment, hiring, and orientation of team members

  • Maintain necessary knowledge and skills to conduct effective patient-centered care coordination activities.

  • Other duties as assigned.


  • NYS Licensed Registered Nurse

  • Two (2) years experience in Outpatient Care Coordination required

  • Experience managing a team of professionals required

  • Experience and Knowledge of Remote Patient Monitoring equipment preferred

  • Knowledge of dual eligible patients (Medicare/Medicaid) preferred

  • Experience with outpatient healthcare setting required

  • BLS Certified

  • Knowledge of CMS guidelines
