$5,000 sign on bonus

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens). Our Certified Home Health Agency and our Gurwin Licensed Home Care Services Agency share an office in Farmingdale, NY.

We are currently seeking a part-time Registered Nurse who will embody the passion and commitment required to deliver the ultimate care for the participants in our Adult Day Care Program. Scheduled hours of work will be 8:30am-4:30pm on Tuesday, Thursday and Friday with possible weekends in the future.
EDUCATION REQUIREMENTS
Graduate of an accredited School of Nursing
Current New York State license to practice as a Registered Nurse
CPR certification preferred
JOB RESPONSIBILITIES
Assumes primary nursing responsibility for the health management of the registrants.
Coordinates the holistic care management of the registrants with the family/caregiver, community physician and interdisciplinary team.
Identifies registrant health needs and provides for appropriate hands-on care and monitoring.
Directs, supervises and instructs CNAs in the clinical aspect of the program.
Cooperates with other disciplines on coordinating direct and indirect services to registrants and families/caregivers.
Supervises registrants medication regime, administers medications and maintains medication administration record as needed.
