Gurwin Jewish Nursing & Rehabilitation Center Therapeutic Recreation Leader Adult Day Care in Commack, New York

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens). Our Certified Home Health Agency and our Gurwin Licensed Home Care Services Agency share an office in Farmingdale, NY.

Our innovative philosophy and growing services offer opportunities to advance your career. We are currently seeking a full-time Therapeutic Recreation Specialist who will embody the passion and commitment required to deliver the ultimate care for our participants.

You will be scheduled to work Monday through Fridays from 8:30am-4:30pm with possible weekends in the future.

EDUCATION AND EXPERIENCE REQUIREMENTS:

BS in Therapeutic Recreation (preferred), working toward a degree or significant experience in a lead therapeutic recreation role in an Adult Day Care setting.

DUTIES AND RESPONSIBILITIES:

  • Capable of identifying and complying with governmental, accreditation, professional and agency standards.

  • Develops and implements specific TR programs for diverse population ie: frail elderly, cognitively impaired, young-physically challenged.

  • Demonstrates ability to plan and organize work to obtain effective use of personnel, m aterials and equipment.

  • Demonstrates the ability to train, supervise, and evaluate paraprofessionals and volunteers.

  • Demonstrates ability to obtain and review pertinent background information about person served, as available from records, charts, from other professional staff and relevant others.

  • Effectively participates in all functions of the biannual Interdisciplinary Care Plan Meeting.

  • Demonstrates ability to develop and document individualized intervention goals and plans which are registrant centered based on assessment, consistent with legal requirements and professional guidelines.

  • Demonstrates ability in monitoring, evaluating and determining effectiveness of individualized treatment plan.

  • Conveys information to treatment team in a timely manner regarding person served.

  • Demonstrates ability in using and maintaining equipment associated with recreation programs.

  • Demonstrates ability in keeping accurate statistical reports of activities offered and level of participation for person served.

  • Demonstrates the ability to respond to concerns of registrants, relevant others, staff and community.

  • Establishes and maintains network with colleagues in community agencies, universities, and allied health professions.

  • Maintains knowledge of current TR trends, techniques, methods, issues, and professional and legal standards.
