What you will do:
- Work in the hospital speaking with patients getting ready for discharge looking for or needing home care and get pre-admission requirements completed.
Have one on one meetings with patients utilizing strong interpersonal and communication skills to complete the pre-admission screening process and obtain consent for services upon discharge
Work as a team player to ensure the best outcomes for our patients
Work closely with the Case Management team and occasionally a hospitalist
Based out of St. Peter's Hospital, but floats to Samaritan campus
Share reasonability for ensuring our most vulnerable population of patients are connected to our Coach and Remote Patient Monitoring Programs
What you will need:
A current license to practice as a Licensed Practical Nurse (LPN) in the State of New York
2 years or more of experience as a LPN
Licensed Practical Nurses must possess valid Basic Life Support (BLS) certification or achieve BLS certification within 90 days of starting via on site education
Valid Driver License and reliable vehicle are required
1 or more years of experience in a similar role
Easily learn new computer applications
Multitask and can accurately prioritize in a fast pace environment
Maintain the highest level of customer service while completing day to day duties and responsibilities
Superior communication skills across individuals with different backgrounds and departments
Critical thinking skills to problem solve
Excellent time management and organizational skills
Engage within a team setting to help promote a strong team culture
Passion to learn, grow and develop in the nursing field
