Job Description
The LPN assists the Medical staff and provides services within the scope of his/her license and training. In addition, the LPN performs clerical tasks. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities.
Essential Accountabilities:
Take the patients vitals and record them as directed.
Draw blood and prepare it for the lab, as directed. Conduct other collection and/or diagnostic procedures as directed within the scope of training and certification.
Under direction of the Physician administer medications.
Assist Registered Nurse with taking inventory of medical supplies.
Engage the patient while medical staff confers with caregiver or other provider.
Make follow-up appointment with patient / caregiver as needed.
Access patients files and provide to medical staff as requested.
Retrieve fax information and provide to medical staff.
Transmit fax on behalf of medical staff, as directed.
Confirm appointments with patient / caregiver as directed.
Escort clients to medical appointments.
Code notes and file into patients files.
Must be able to utilize a computer.
Must be able to utilize business machines, including fax and copier.
Must be able to transport self to varied sites/ clients residences.
Participate in team meeting, as requested.
Able to lift/carry patient files, packages and boxes of supplies.
In an emergency, must be able to physically assist with the evacuation of patients.
Must be able to climb and descend interior and exterior stairs.
Maintain confidentiality at all times consistent with HIPAA and other applicable policies.
Work schedules and locations subject to change.
Comply with all policies and procedures, including those regarding client rights and incident reporting.
If appropriate, act as a translator for patient.
This does not exclude the assignment of duties or exclude the performance of other duties, not mentioned, as assigned by supervisors.
Must be able to communicate information verbally and in writing for the purpose of taking a message, conveying information or transmitting instructions.
Must be able to read typed and written correspondence, clinical notes, communication log, minutes of meeting and doctor orders.
Report to the team leader.
Job Qualifications
License and certification in New York State as a Licensed Practical Nurse (LPN).
