But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
The Outreach Coordinator will work in a 24/7 S treet O utreach P rogram that addresses homelessness and related activity. R eport ing to the Program Director , t he Outreach Coordinator will supervise four Team Leaders and will be responsible for all outreach activity in their respective borough. Responsibilities include oversight of 24/7/365 outreach services, scheduling, regular supervision, performance planning, managing 311 response, and engagement with street homeless individuals. The O utreach Coordinator will ensure that anyone who is homeless is assessed and will connect people with appropriate services to help overcome homelessness. The Outreach Coordinator will play a prominent role in executing formal census counts and together with other program leadership, will maintain excellent working relationships with community partners including local NYPD precincts, BIDs, Community Boards, hospitals, etc. The O utreach Coordinator will be responsible for frequent collaboration with the Clinical Coordinators on client-related issues and maintaining strong and open communication to mutually ensure that client needs are being met.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Provide supervisory direction, leadership and supervision to Team Leaders and Outreach Housing Advocates
Collaborate with other program leadership to meet program goals
Conduct outreach as needed, engaging and assessing street homeless adults
Oversight of 311 response
Oversee and participate in Code Blue/Red Alerts
Coordinate delivery of care with other agencies
Create and implement tracking tools as needed
Participate in CB meetings, NYPD roll call, BID meetings, etc.
Oversee encampment clean up’s and eradicating panhandling and other activities
Performs other related duties as assigned
Minimum 5 years in a supervisory capacity working with homeless or disabled populations.
Preferred training in CPR & First Aid, Opiate Overdose Prevention, crisis intervention, de-escalation, Harm Reduction, Motivational Interviewing, Mental Illness, Substance Abuse
Strong supervisory skills, including previous experience managing a 24/7 operation & staff.
Solid verbal and written skills. Excellent clinical and assessment skills
Bilingual Spanish/English is preferred
Excellent computer skills, including proficiency in Microsoft Word and Excel. Familiarity with Access-based databases and the ability to learn new programs are preferred
Must have valid NY driver’s license with clean driving record
Must be willing and able to work in challenging conditions as needed ( i.e. inclement weather, remote areas of parks, bridges, etc.)
Must be willing and able to be on call 24/7 and to come on site as needed during any shift
