Under supervision of the Hospice Team Manager and AVP Operations, act as assistant to the Hospice interdisciplinary group members and coordinate the provision of ancillary services. Performs initial and ongoing assessment of the patients, interprets data collected and takes appropriate interventions. Applies hospital patient care standards, nursing practice standards and evidence-based practice to patient care activities.

Job Responsibility

  • Order laboratory, x-ray, medical supplies and equipment for patients and document. Accurately enters all ancillary services information into the Suncoast EMR. Fax lab results to patient's primary care physician and relay information to Team Manager and Hospice Care RN for follow-up. Post results in EMR. Contact patient /family to verify equipment and/or supply delivery. Process new patient admissions; existing patient discharges/transfers and deaths. prepare agenda for Interdisciplinary Team Meetings and attend meetings to scribe. Assist Team Manager and IDT members with various miscellaneous assignments. Provides input into the development and integration of the patients interdisciplinary plan of care; prepares patient and family for discharge and follow-up care.

  • Assesses acute and chronic pain using hospital approved pain scale; manages pain to patient's reported acceptable level of comfort.

  • Reassesses pain management after intervention and documents effectiveness using hospital approved pain scale; responds to patient/family needs and customer concerns as soon as possible and compassion and understanding.

  • Participates in monitoring patient care standards; schedules patient care assignments for various employees, both professional and non-professional.

  • Evaluates the performance of employees assigned to the unit and makes recommendations concerning such performance; guides patient care staff in accomplishment of goals of care on a daily basis and during emergencies.

  • Guides nursing staff to develop, execute and evaluate the plan of care for each patient in collaboration with other members of the health care team; plans for the implementation of new techniques for the care of patients in the operating room with Nurse Educator.

  • Confers and collaborates with the Educator in planning clinical orientation and skill training for new staff members; regularly participates in the evaluation of new products, equipment, procedures, data collection and research projects.

  • Identifies budget needs required to support and implement new programs in area of responsibility; interprets and implements safety standards and continually monitors the environment for potential and/or actual hazards, reporting problem areas promptly to nurse manager.

  • Monitors the maintenance, cleanliness and orderliness of work and storage areas, fixtures, furniture and equipment of the unit.

  • Participates in departmental quality initiatives; assumes responsibility as charge nurse when assigned.

Job Qualification

  • Associate's Degree preferred, or equivalent combination of education and related experience.

  • 1-3 years of technical experience, required.
