But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

TheCase Manager provides case management services to chronically homeless men, some with multiple disabilities and dual diagnoses in order to support the individual in their transition to permanent housing. The case manager within the Safe Haven accomplishes clients' care by assessing treatment needs; developing treatment plans and evaluating progress while facilitating interdisciplinary approaches that are tailored to each client. All interactions with clients are clinically driven with the goal of improving the clients’ wellbeing. The case manager consistently offers clients opportunities to improve their wellness. This position is Sunday - Thursday


• Conduct an initial assessment and intake interview with incoming clients who are referred by the outreach system

• Conduct psychosocial evaluations

• Connect clients to psychiatric services

• Provide motivational interviewing and harm reduction counseling to individuals who engage in high risk behavior

• Appropriately connect and motivated clients to medical treatment & substance use treatment facilities when applicable

• Help clients obtain identification, public benefits and other applicable entitlements

• Meet at minimum weekly with all clients to assess needs, encourage harm reduction, and to develop a collaborative service plan

• Help clients determine their own next steps

• Prepare all documentation for housing applications; assist and counsel clients in the process of obtaining permanent housing

• Maintain record keeping in compliance with agency standards as well as those of City and State agencies; manage caseload of approximately 18 clients and facilitate weekly groups

• Escort clients to appointments when needed

• Engage clients in multi-disciplinary case conferences to overcome their internal and external obstacles

• Crisis intervention and de-escalation to help reduce the harmful consequences of risky behaviors

• Manage clients schedules to ensure program adherence

• Performs other related duties as assigned


• Must be flexible with varying work hours and shifts

• Bachelor’s Degree or equivalent experience

• The case manager must possess an advanced understanding of the safe haven model as well as the harm reduction treatment model and theory

• Expertise in the harm reduction model, safety contracting, motivational interviewing and de-escalation interventions

• Three-plus year of case management experience preferred

• Experience with navigating duel case management systems such as Foothold Technologies and CARES

• Expertise in navigating Microsoft Office, Microsoft Outlook Calendar; utilizing the calendar to organize schedule

• Expertise in writing comprehensive psychosocial assessments, detailed progress notes, and goal-oriented service plans

• Strong collaboration skills with multiple service providers and collaterals; internally and externally

· Demonstrate good leadership and excellent professional communication skills

· Excellent verbal and written communication skills with strong documentation skills

· Experience working with homeless adults and substance use preferred

· Valid NY license with driving record that is in good standing. Maintaining a good driving record (as defined by the organization) is a condition of employment
