Services for the UnderServed, Inc (SUS) Outreach Specialist in New York, New York

Location: East Harlem, New York

Schedule: Monday - Friday 9 am - 5 pm, heavy field work

The salary min $62,000 to max $62,000.

SCOPE OF ROLE:

The Outreach Specialist will be part of a clinical team of “can-do” people who continually challenge the status quo, merging cutting-edge innovation with groundbreaking research to decrease disparities in mental health and substance use problems by utilizing technology and training to provide high quality and accessible mental health assessments, immediate care, and referrals, as well as promote financial wellness among vulnerable populations. The Outreach Specialist will be based at an integrated OASAS and Article 31 Outpatient Mental Health Clinic serving adults living with substance use disorders and mental illness; including clinical program development, supervision, training and evaluation of professional staff.

ESSENTIAL DUTIES:

  • Undergo training by the New York State Psychiatric Institute (NYPSI) Mental Wellness Equity Center.

  • Utilize the Mental and Financial Wellness Everywhere Tool (mwTool), to screen clients for symptoms of common disorders (depression, anxiety, PTSD), severe disorders (psychosis, mania), suicide, and financial stressors.

  • Based on mwTool results, match the type and severity of symptoms to one or more brief low-intensity counseling interventions or coordinate referral to a higher level of care (mental health specialist).

  • Offer and deliver brief structured counseling interventions with step-by-step guidance from provider facing apps specific to the client’s symptom severity and areas of need.

  • Provide direct coaching, education, and advocacy in linking, engaging clients to get services through referrals in the community.

  • Participate and complete all required training as indicated by S:US and NYPSI.

  • Form relationships with community-based organizations (CBOs) to engage with communities with long histories of barriers to care.

  • Attend morning huddles to communicate impediments and confirm arrival of outreach locations.

  • Maintain frequent communication with your supervisor via Microsoft Teams or other communication platforms.

  • Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by S:US' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with S:US policies, OMH and OASAS regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services.

  • In coordination with relevant members of the SUS management team, conducts outreach and marketing to potential referral sources internal and external to SUS; active liaison with community service providers, hospitals, and other prospective referral sources is may be required.

  • Coordinates care with SUS nurse practitioners, psychiatrists and administrative staff; closely coordinates with clinicians external to SUS as appropriate.

  • Assist in managing client crises and onsite and offsite emergencies.

  • Perform other tasks as assigned.

REQUIRED EDUCATION AND EXPERIENCE

  • Master’s degree in social work, psychology, rehabilitation counseling, or a related field or licensure/registration as a registered nurse preferred. Bachelor’s degree in social work, psychology, rehabilitation counseling, or a related field required.

  • Must have excellent verbal, written and interpersonal communication skills.

  • Proficient in MS Word/Excel. Ability to research material via the internet

PREFERRED QUALIFICATIONS & SKILLS

  • Professional experience working in a multi-disciplinary mobile team.

  • Bilingual English/Spanish
