Visiting Nurse Service of New York Registered Nurse, Behavioral Health in New York, New York


Responsible for providing high quality nursing care to patients with psychiatric or psychological problems consistent with Agency philosophy, Agency policy and Standards of Nursing Practice.

Conducts health and psychosocial assessments of patients and develops treatment recommendations.
Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
Transports and utilizes required medical equipment and supplies using VNSNY designated/supplied carrying case weighing approximately 25 - 30 lbs. to and from patient homes/care facilities, VNSNY offices and other locations.
Provides nursing care in accordance with Agency policies, practices, procedures, Standards of Nursing Practice, and Assertive Community Treatment Program guidelines.
Provides any of the following services to patients: advocacies, wellness self-management, harm reduction treatment, monitoring of mental status, emotional support, brief psychotherapy, family based interventions and crisis intervention.
Provides health education to ACT clients and consultation and support to ACT staff, providers, families, and significant others.
Implements treatment plans, revising and updating as necessary, in collaboration with the ACT team.
Manages all aspects of the psycho-pharmacological treatment of ACT clients, in collaboration with, and under the direction of, the ACT MD.
Makes appropriate community referrals for services as required.
Participates in interdisciplinary team meetings, case conferences and clinical supervision.
Attends in-service education programs.
Assumes responsibility for continued professional growth.
Maintains productivity sufficient to meet ACT Team standards.
Maintains records in accordance with OMH, DOH/MH and VNSNY record keeping standards.
Performs other related duties and projects, as required.
Works under moderate supervision of the Program Manager.


Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required. Valid drivers license may be required, as determined by operational/regional needs.
Education: Bachelors degree in nursing from a state approved diploma program, OR, Associates degree with at least one year of experience as a full-time staff nurse in a medical-surgical nursing area required. Masters degree in Psychiatric Nursing preferred.
Experience: Minimum one year experience in a Homecare environment or recent graduate of a nursing internship program required. For candidates with a Nursing Diploma or Associates Degree in Nursing, a minimum of two years of recent inpatient, outpatient or home care psychiatric experience OR successful completion of a Psychiatric Nursing Clinical Certification program or psychiatric nursing school program required.
For candidates with a Bachelor's degree in Nursing or Master's Degree in Psychiatric Nursing, a minimum of one year experience in a mental health or home care psychiatric setting OR successful completion of a Psychiatric Nursing Clinical Certification program or psychiatric nursing school program required. Bilingual skills may be required, as determined by operational needs.
