Social Worker (Shelter)

The Social Worker is responsible for providing psychosocial services, including screening, outreach, risk/psychosocial assessment, crisis intervention, determination and referral to follow-up, short term counseling, and psychoeducation to a diverse youth homeless population. The Social Worker will also serve in an informal capacity as a consultant/supportive coach to residential staff regarding young people's psychosocial and mental health needs to support overall program success. Reporting to and receiving clinical supervision from CHNY's Clinical Administrator (LCSW), the Social Worker will support a culture that successfully achieves meaningful results for the young people we serve.


(Responsibilities to include but not limited to the following)

  • Provide (weekly or as needed) short term individual and group counseling (and psychoeducation) to youth experiencing homelessness to address mental health and psychosocial needs (per service plan). This includes short-term crisis intervention, including advocacy and follow-up for designated caseload.

  • Completes psychosocial assessments (e.g., screening for mental health and substance use concerns and mental status exams) for higher need residents. Identify current mental health, social, and emotional needs, making initial recommendations, and referrals.

  • Integrate evidence-based programs such as Screening, Brief Intervention, and Referral to Treatment (SBIRT) used to identify, reduce, and prevent problematic use, abuse, and dependence on alcohol and illicit drugs. Covenant House will provide training.

  • Integrate evidence-based treatments such as; Motivational Interviewing (MI), Safe Crisis Management (SCM), and skills based on Dialectical Behavior Therapy (DBT) that address emotional and behavioral difficulties associated with single, multiple, and complex-trauma experiences. Covenant House will provide training.

  • Actively link young people who are interested in longer-term mental health care and psycho-social supports to community-based providers and communicate with providers/social workers at referring agencies as necessary.

  • Work in small teams with assigned Case Managers to coordinate and collaboratively address the case management and social work needs of youth.

  • Develop and co-facilitate innovative support/psycho-education/skills building groups related to topics such as Anger and Stress Management, Substance Abuse Prevention, Affective Regulation, and Healthy Relationships.

  • Co-facilitate mental health-focused and supportive workshops and training for both young people and agency staff.

  • Work collaboratively with all internal and external service providers.

  • Provide consultation to Case Managers and residential staff'providing assessments, information (regarding issues around trauma, mental health, and substance use), and referrals as needed.

  • Document and complete all assessments and reports, including psychosocial assessments, treatment plans, session notes, referrals, and follow-up in a timely manner in accordance with good professional practice and agency policy.

  • Ensure that paper files are grant, regulation, and government agency compliant at all times.

  • Ensure that all documentation is completed in ETO and ECW in a timely manner (including progress notes and treatment plans for billing purposes).

  • Meet regularly with clinical supervisors to discuss concerns, reflect on practice and client needs, and carry out plans for young people.

  • Participate in weekly case review and daily shift transition.

  • Flexible schedule required, sometimes including holidays and weekends, as needed.

  • Always reflects CHNY's values and principles on social media.

  • Performs other duties as assigned/needed.


  • NYS License in Social Work (LMSW) required.

  • LMSW must be actively pursuing LCSW designation and must achieve such designation within 3 years of the hire date.

  • The provider must be eligible to bill NYS Medicaid for services, either directly or under qualifying supervision in pursuit of licensed clinical social work status by the New York State Education Department.

  • Provider must provide documentation annually to Human Resources reflecting pursuit of LCSW designation.

  • Experience providing counseling and casework with at-risk youth highly preferred; Internships in this field to be considered.

  • Knowledge of trauma and mental health issues facing multi-stressed youth and families.

  • Experience working in a shelter or residential setting preferred.

  • Knowledge of external and social service systems.

  • Solid assessment, interviewing, and crisis intervention skills.

  • Flexible work schedule to include evenings, weekends and holidays.

  • Bilingual English/Spanish preferred
