EAC Network seeks a Part Time Bilingual Case Manager for the Child Advocacy Center (CAC) Suffolk Mutli-disciplinary Team Enhancement Initiative (MDTEI) program located in Riverhead, NY. The position is PT, 28-30 hours per week.
Primary Purpose of Job:
The bilingual CAC Case Manager is responsible toprovide direct service to child victims of abuse to meet OVS requirements and focus on the needs and the functioning of the satellite CAC as it pertains to Multidisciplinary Team coordination and efforts.
Principal Duties & Responsibilities:
Provide crisis intervention to families
Provide outreach and case management services to crime victims
Conduct assessments for client needs and develop a plan to meet such needs.
Transport and escort clients to court proceeding, medical appointments and other appointments as needed.
Provide assistance to victims by submitting applications toNYS Compensation Board, police reports, restraining orders, safe housing and financial benefits.
Provide information about the criminal justice process
Make referrals to other victim service programs, supports, and resources
Document in the CAC database the services required by the client to ensure accurate case records are maintained.
Maintain accurate database of professional referral resources that assist stated population
Assist with MDT training development and delivery
Conduct assessment of MDT relationships, satisfaction, level of participation, etc. and
MDT Memorandum of Understanding (MOU) and Working Protocol review, revisions, and re-execution as necessary
Prepare monthly and quarterly statistical and annual reports as required by funding source.
Knowledge, Skills, & Abilities Required:
Bachelors Degree required, MSW preferred.
Bilingual (Spanish Speaking) required.
Minimum of one year of relevant experience.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
Excellent communication, organizational and interpersonal skills.
Must have access to automobile and possess valid NYS drivers license.
