Job Description:

Provides outreach, engagement, support, and counseling to individuals transitioning from inpatient hospitalization to more independent community living. Interacts independently with clients one on one in clients’ homes and in community settings utilizing knowledge of mental illness, substance use, physical health, safety and sound judgment. Maintains an organized caseload of frequently changing individuals in engagement status. Partners with clients in pursuing Service Plan goals through coaching, teaching, and counseling within the scope of the program’s services. Teaches, supports and assists with activities of daily living/functional performance including: personal hygiene, maintaining safe and sanitary living area, meal planning, money management, daily scheduling, community integration, healthy behavior and symptom management, medication management, self-advocacy, and rehabilitation skill building. Assists clients with maintaining ongoing contact with care providers, following through with appointments and treatment recommendations and informing appropriate providers of outcomes. Collaborates with other East House staff and external providers to coordinate client services. Manages time efficiently and effectively to maintain accessibility to clients. Maintains familiarity with all clients through participating in Case Reviews, reviewing communication logs, etc. Maintains documentation to set standards. Position involves travel within Greater Rochester area and five surrounding counties. This program is licensed by NYS OMH.

Hours:Tuesday through Saturday 8:00 AM to 3:30 PM


  • A high school diploma and 3 years of human services work experienceOR

  • An associate’s degree plus two years human services experienceOR

  • A bachelor’s degree and 1 year human services experienceOR

  • An equivalent combination of education and experience.

  • All employees are required to have a valid NYS motor vehicle license, safe driving record, and an insured personal vehicle for use at work.


Wages are determined based on job-related education and experience.
