Description

The health services that became theAnthony L Jordan Health Corporation (http://www.jordanhealth.org/) (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.

Jordan Health is now seeking aQI Leadwho, under the guidance of the Director of Quality Assurance, is primarily responsible for measuring quality as well as identifying opportunities and strategies for clinical performance improvement. Under the direction of the CMO (or designee), she or he facilitates the organization’s Quality Improvement practices.

Requirements

The QI Lead will ensure to:

· Provide clinical data reporting for analysis, facilitating workflow, regulatory, quality of care, outcomes and business objectives using various systems such as eClinicalWorks (HER system), BridgeIT, Microsoft Office Suite of tools and third-party insurer data resources (as provided by insuers).

· Provide clinical data reports for grants/programs of AJHC for the CMO. Some include Health Resource Services Administration HRSA) metrics, Uniform Data System (UDS) measures, Provider Dashboards, Patient Centered Medical Home (PCMH) and Meaningful Use.

· Create and administer feedback mechanisms on data reports to providers and other clinical staff, as directed by CMO.

· Analyze results, draw conclusions, and create presentations using available technologies.

· Make recommendations for support improvements and assist staff to integrate changes into day-to-day workflow redesign when needed.

· Present data to various committees, workgroups and outside community agencies as requested by CMO, DON, CEO or other team member.

Education And Experience Required:

· Associate Degree or working on the degree.

· Combination of education and relevant work experience will be considered.

· High level of work knowledge and understanding of the Microsoft Office suite of software, specifically Excel, Word, and PowerPoint.

· High level of work knowledge and understanding of eClinicalWorks or other electronic health record HER.

· Experience with other electronic reporting tools such as BridgeIT, Phytel.

· Healthcare background, experience and performance that promotes a high level of credibility with clinical professionals.
