Deaf Wellness Clinic

Description of Patient Population:

    The social worker assigned to the Deaf Wellness Clinic provides psychosocial assessment and psychiatric diagnosis and treatment for emotionally disturbed adult patients 18 years and older. The social worker functions as primary therapist and is responsible for the overall coordination and implementation of the treatment plan for patients assigned to his/her caseload. The social worker participates on an interdisciplinary treatment team and develops liaisons with health and social agencies in the community, as well as with other health professionals within the hospital to support evaluation and treatment of the patient. The social worker also provides treatment, and crisis intervention as appropriate.


    Administrative (5%)

  • Participates in the development of a supervisory contract and in annual performance evaluation.

  • Attends Social Work Division and Program Staff Meetings.

  • Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including timely completion of reports/forms, time reporting and other Division forms; Hospital and Division mandatory inservice training programs and UHS reviews.

  • Subscribes to the Departmental of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate to job duties.

    Clinical (95%)

  • Performs psychosocial assessment and diagnostic formulations for patients and, when indicated, their families.

  • Provides social work treatment to patients and families within Deaf Wellness Services

  • Ensures the development and implementation of comprehensive and individualized treatment plans for patients

  • Provides screening, evaluation, individual, family and group treatment.

  • Functions collaboratively and consultatively as member of interdisciplinary team.

  • Coordinates with other resources in the community and within SMH to ensure continuity of care.

  • Documents treatment in patient's electronic medical record in compliance with Division, Department and Hospital policy.

  • Participates in on-call activities of the Program.

  • Provides back up social work services as requested during the absence of certain co-workers.

    Teaching (2.5%)

  • The social worker may provide supervision and training to social work students as determined by the social work supervisor in consultation with the Clinical Manager for Social Work and the Director of Ambulatory Services.

  • The social worker may participate in teaching concerned with issues of patient and family assessment, and treatment interventions as they relate to the psychosocial aspects of patent care within the Adult Ambulatory Clinic.

    Research (2.5%)

    The social worker may participate in interdisciplinary and/or social work research endeavors as determined by the social work supervisor in consultation with the Clinical Manager for Social Work and the Director of Ambulatory Services.



    The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).


  • The social worker is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.

  • The social worker is accountable to the Director of Ambulatory Services for providing professional social work services in accordance with the policies and standards established by the Service and the Department of Psychiatry.


    Social work evaluations are conducted annually by the social worker's supervisor, with commentary from the Clinical Director of the Service or Team Leader. Salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester .


    The social worker that occupies this position must have a MSW from an accredited social work graduate program. They must meet the qualifications to be a licensed Social Worker within eighteen months of employment. Further, this social worker must be fluent in ASL and good working knowledge of Deaf culture.


    This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Director of the Service, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of the Service. Any position description modifications are authorized by the Social Work Division Clinical Manager for Psychiatry.
