$18/hour with no benefits or accruals.Must be able to work at least 12 hours per week up to a maximum of 28 hours per week.
Shifts available include Sunday - Saturday. Must be able to commit to working at least a Saturday or Sunday each weekend.
Provide a home-like atmosphere conducive to habilitation, advancement and the well being of the individual.
Assist each individual in acquiring basic activities of daily living, social, recreational, and communicative skills for supervised living and functioning within the community.
Assist each individual to achieve his/her maximum growth as a human being and set attainable goals, consistent with his/her potential and treat them with dignity and respect.
Serve as a member of the residence treatment team, along with other members of the interdisciplinary team; assist in formulating an individual habilitative plan for each individual.
Provide quality care assurance and comply with rules and regulations governing them.
Assess, instruct and assist individuals as indicated or necessary in activities of daily living, personal, social and health habits including, but not limited to: self-care, personal hygiene, cleanliness, maintaining personal clothing, dressing, undressing, eating, tidiness of self and living area, bathing, toileting, community integration, money management and other skills as necessary for community living. Document needs and progress regarding same. Must be able to perform the above tasks which require lifting, positioning and transferring without restriction.
Implement service/treatment plans to aid the individuals in learning about the community and use of community facilities to the extent feasible.
Participate in individual habilitative planning and implementation.
Complete entries into a daily log and documentation of individual's progress or service/treatment needs.
Ensure that all incident, emergency and safety procedures and regulations are followed.
Evaluate storage and administration of prescribed medications for individuals in accordance with physician's recommendations and maintain medication sheets; report all untoward reactions to the appropriate person(s).
Make known any accidents, illness, behavioral or emotional problems involving the individuals, and other unusual circumstances to the appropriate person(s) and record documentation regarding same.
Track and document medical appointments, employment, day programming/treatment and personal business of the individuals; provide transportation when necessary, and as circumstances warrant.
Maintain responsibility in performing household duties such as: laundry, house cleaning, cooking, as well as the procurement, storage and serving of food.
Implement all service plans in a consistent manner and interact with individuals regularly in a professional manner while maintaining a homelike atmosphere consistent with indicated treatment plans.
Provide recreational services on an individual and group basis and provide documentation regarding same.
Be available and arrange for emergency services for individuals including remaining at the residence, if necessary, beyond regularly scheduled hours to cover emergency situations, such as an individual's illness, inability to attend day program due to behavioral or health problems, critical staff shortages, etc. Need will be determined by the Res. Hab. Manager or his/her designee.
Notify day program of an individual's absence or incidents in which the individual was involved since last attendance at program. This notification will be by phone or written correspondence.
Maintenance of the residence, which includes making minor repairs when necessary, notifying Res. Hab. Manager (or beeper during off hours when there are emergency situations, when major repairs to building, equipment or furniture are needed, and assisting in housekeeping duties with residents whose skills require refinement.
Participate in meetings as appropriate to facilitate communication involving individual's needs, counseling strategies, service/treatment plans, progress and other pertinent information.
Adherence to agency Personnel Policies and Practices.
Supervision of individual's use of personal monies and accounts and obtain appropriate receipts regarding same.
Other duties as may be assigned, or required by the needs of the residents and/or program.
Must become Medication Certified for certain program sites upon hire.

Must be at least 18 years old, have a valid NYS Driver's license and HS Diploma or GED.
Must be currently certified in all OPWDD trainings. (If you are not currently certified you would be required to attend our paid orientation in order to get certified.)
