Brookhaven Memorial Hospital Medical Center Certified Recovery Peer Advocate - Part Time in SHIRLEY, New York

Outpatient Substance Use Services provides clinical and supportive care to a diverse population of patients, utilizing a clinical team that provides assessment, evaluation, treatment, medication assisted treatment, education and referral. The peer is an integral member of the Multidisciplinary Team who provides ongoing support, encouragement, and assistance with linkage and follow-through for patients. The Peer will possess strong interpersonal skills and have keen ability to navigate through the community and healthcare resources for the benefit of the patient and their family/significant other.

  • Provides peer support and develops a positive rapport with patients and or their families/significant others.

  • Meets with patients in a variety of settings, including the emergency room, and community settings.

  • Assists patients with follow through for OP appointments and securing other community resources.

  • Consistently ensures patient dignity and respects patient values and operates within ethical standards.

  • Demonstrates the ability to communicate needs of patients with treatment team

  • Communicates effectively with families and agencies, conveying a positive, professional image.

  • Conducts educational sessions, displaying good theoretical knowledge of subject matter and effective communication skills in presenting material.

  • Provide services for the purpose of outreach and engaging individuals into treatment, or reinforcing current treatment

  • Assists in advocating for client driven goals and objectives

  • Assists clients in developing outside recovery supports

  • Preferred Active Certified Recovery Peer Advocate certificate (CRPA-P considered)

  • Preferred 500 hours of work experience with 25 hours of supervision by qualified supervisory staff (volunteer experience considered)

  • Approximately one to three months on-the-job experience necessary in order to become familiar with local health and welfare resources and facilities and health center policies and procedures

  • Ability to engage patients and develop rapport

  • Good interpersonal skills demonstrable in interactions with staff, administrators, patients, and visitors

  • Continued education through related seminars, patient review meetings, workshops, etc. Participation in departmental meetings, in services and attention to hospital center memoranda.

  • Good follow-up skills to ensure patient engagement

  • Lived experience in mental health, substance use, and criminal justice and the willingness to disclose lived experiences
