Unity House ESSHI Case Manager in Troy, New York

Responsibilities:
Provide supported service to individuals living with mental illness.
Provide support for the Empire State Supportive Housing Initiative.
Provide independent living education.
Advocacy for consumer on caseload.
Assist with housing placement and referrals. Crisis intervention to program consumers.
Coordinate care with other case managers and providers to wrap around services.
Promote a culture of inclusion and diversity.
Requirements: A high school diploma or GED and 2 years experience working with adults living with a mental illness. Valid New York State Driver License and acceptable driving record as determined by our company’s liability insurance policy. Reliable transportation.
